SAHPRA Medicine Safety Alert

The South African Health Product Regulatory Authority (SAHPRA) has been alerted to the inadvertent administration and/or medication errors where tranexamic acid was erroneously administered in place of bupivacaine injection due to similarities in packaging. This has resulted in some patients incurring post-administration complications, and in some cases, fatal outcomes.

SAHPRA have issued a Medicine Safety Alert and urge practitioners to report any adverse drug reactions (ADRs), or product quality problems to SAHPRA via:

  • Complete the ADR reporting form accessible via the SAHPRA website and email it to adr@sahpa.org.za.
